The type of personal information we collect
We currently collect and process the following information:
- Personal identifiers, contacts and characteristics (for example, name and contact details)
- Details relating to your medical history and findings
How we get the personal information and why we have it
Most of the personal information we process is provided to us directly by you for one of the following reasons:
- To find out about your medical concerns and how we can help you as a medical provider
- To contact you about appointments, newsletters and promotions
We may also receive personal information indirectly, from the following sources in the following scenarios:
- From your GP or medical provider, if required. We will always consult you first if we need to request additional medical information to help in you care.
We use the information that you have given us in order to help in the diagnosis and management of your health needs.
We will never share your data with anyone who does not need access without your written consent. The only people who will have routine access to your data are your practitioner(s), in order that they can provide you with treatment, and our reception staff, who organise appointments and reminders, and can act as an intermediary between you and your practitioner(s).
It is good practice to liaise with other healthcare professionals, but we will seek separate consent for your permission to write to your GP, or to communicate with any other healthcare professional(s).
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:
(a) Your consent. You are able to remove your consent at any time. You can do this by contacting the clinic director, Jake Neil.
(b) We have a legal obligation.
(c) We have a vital interest in providing the service you have sought from us.
(d) We have a legitimate interest in understanding your medical needs to the best of our ability and provide appropriate care.
When you supply your personal details to the clinic, they are stored and processed for 4 reasons:
1. We need to collect personal information about your health in order to provide you with the best possible treatment. Your requesting treatment and our agreement to provide that care constitutes what the law calls a “Contract”.
2. We have what is termed a “Legitimate Interest” in collecting that information, because without it we could not do our job effectively and safely.
3. We also think that it is important that we can contact you in order to confirm your appointments with us or to update you on matters related to your medical care. This again constitutes “Legitimate Interest”, but this time it is your legitimate interest.
4. Consistent with our role as healthcare professionals, we may occasionally send you general health information in the form of articles, advice or newsletters. This again falls under the category of “Legitimate Interest”.
5. In order for us to keep accurate financial records and produce receipts and invoices for your visits, your basic contact details will be stored on our secure accountancy software. The data kept will also include your payment history but will not include medical notes.
How we store your personal information
We have a legal obligation to retain your records for 8 years after your most recent appointment (or age 25, if this is longer), but after this period you can ask us to delete your records if you wish. Otherwise, we will retain your records indefinitely in order that we can provide you with the best possible care should you see us at some future date (or need us to transfer your records to someone else). Your records are stored electronically on a secure server which is backed up regularly. Access to all data is password protected. Any paper documents are scanned onto our system before being securely shredded.
Your data protection rights
Under data protection law, you have rights including :
- Your right of access – You have the right to ask us for copies of your personal information.
- Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
- Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances.
- Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal information in certain circumstances.
- Your right to object to processing – You have the the right to object to the processing of your personal information in certain circumstances.
- Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.

Complaints
We want you to be absolutely confident that we are treating your personal data responsibly, and that we are doing everything we can to make sure that the only people who can access that data have a genuine need to do so. If you feel that we are mishandling your personal data in some way, you have a right to complain. Clinic Director Jake Neil is responsible for data protection, and can be contacted at the clinic by any of the usual methods, or by email.
